If you find any damage to your SUPER VAC VF124 Ventilator during shipment, follow these steps to file a claim:
1. Call your transportation agent immediately and have them make a description of the damage on the freight or express bill.
2. Write to SUPER VAC and request the original bill of lading.
3. Obtain a claim blank from the express or truck company and fill it out.
4. Attach the claim form to the original bill of lading, along with a copy of the invoice and a memo showing the value of the damaged goods.
5. Submit these documents to your local transportation agent.
6. The transportation company will process your claim with reasonable promptness.
Note: SUPER VAC cannot file claims for damages on your behalf. It is recommended that you file the claim directly to expedite the process. Keep all damaged goods for examination by the truck or express company inspector, as disposing of them may result in your claim not being paid.
