To align and adjust the Fillauer Element DS prosthetic foot, follow these steps:
1. Proximal Attachment: Attach the foot to an ISO 10328 compliant, Fillauer or equivalent, standard adult pyramid receiver using the specified torque setting. Use proper thread locker for final delivery.
2. Static and Bench Alignment: Use standard bench alignment techniques. Establish the initial heel height of ⅜ inch or 1 cm using a spacer. Ensure the top of the pyramid is parallel to the work surface. A backward leaning pylon indicates a low heel height.
3. Transtibial Bench Alignment: Set the socket with the appropriate amount of inset. The plum line from the bisection of the socket at the proximal brim should bisect the ankle pyramid. The foot may be slightly inset depending on limb length.
4. Transfemoral Bench Alignment: Follow the alignment instructions provided by the manufacturer of the prosthetic knee in use.
5. Dynamic Alignment: After static alignment, make small adjustments to optimize gait and efficiency. Consider patient feedback during this process. Check for smoothness of gait and ground contact during stance phase. Adjust the foot stiffness, heel height, and support as needed.
6. Special Considerations for DuraShock Component: Ensure proper external rotation of the foot in relation to the socket. 7. Bumper Replacement: If the heel bumper needs to be moved or replaced, remove it using heat and solvent. Adhere the new bumper using cyanoacrylate instant adhesive. Do not place the bumper more anteriorly than the main foot bolt.
8. Changing Toe Stiffness: The toe stiffness can be adjusted by changing the amount of plantar/dorsi-flexion of the foot. Contact Fillauer for assistance if a smooth stance phase cannot be achieved.
