To align the fillauer Aeris Performance 2 prosthetic foot, follow these steps:
1. Proximal Attachment: Attach the foot to an ISO 10328 compliant, Fillauer or equal, standard adult pyramid receiver using the specified torque for set screws (15 N·m for Fillauer components).
2. Static and Bench Alignment: Use standard bench alignment techniques. Establish the initial heel height of ⅜ inch or 1 cm using a spacer under the heel. Ensure that the top of the pyramid is parallel with the work surface. A backward leaning pylon indicates a low heel height, while a forward leaning pylon indicates a high heel height.
3. Transtibial Bench Alignment: Set the socket with the proper amount of inset. A plum line from the bisection of the socket at the proximal brim should bisect the ankle pyramid. The foot may be slightly inset, 1–12 mm, depending on the limb length. The longitudinal axis of the foot will be outset approximately 5° by aligning the medial border of the foot with the line of progression.
4. Transfemoral Bench Alignment: Follow the alignment instructions provided by the manufacturer of the prosthetic knee in use.
5. Dynamic Alignment: The Aeris Performance 2 foot is flexible and conforms to uneven terrain. Make small alignment changes to optimize gait and ensure a smooth transition from heel to toe. Patient feedback is essential during this process. Adjust the plantar and dorsiflexion angles to achieve a smooth transition. The pylon should remain vertical in the frontal plane throughout gait.
6. Procedure: Check for smoothness of gait and ground contact throughout the stance phase of gait. Make adjustments to the socket position or foot alignment as needed to correct any issues with tibial progression, heel compression, or foot progression. Ensure that the pylon remains vertical.
